What Is Diabetes, and Why Is Exercise Essential for Management?

Diabetes is one of Australia’s most prevalent health conditions. This comes as a result of a modern lifestyle detailing limited incidental movement, and easy access to excessive amounts of food, particularly sugar.

One of the most powerful tools available for managing diabetes is exercise. Regular movement can improve blood glucose control, reduce the risk of complications, and enhance overall quality of life.

What Is Diabetes?

Diabetes is a condition that affects the body’s ability to regulate blood glucose (blood sugar).

Glucose is the body’s primary source of energy, but it requires the hormone insulin to move from the bloodstream into cells where it can be used.

When insulin production or insulin function is impaired, blood glucose levels rise, increasing the risk of both short-term symptoms and long-term health complications.

Understanding the Different Types of Diabetes

While diabetes is often discussed as a single condition, there are several different forms.

Type 1 Diabetes

Type 1 diabetes is an autoimmune condition in which the immune system attacks the insulin-producing cells of the pancreas.

As a result, the body produces little or no insulin and lifelong insulin therapy is required.

Type 1 diabetes is not caused by lifestyle factors and can develop at any age, although it is commonly diagnosed during childhood or early adulthood.

Type 2 Diabetes

Type 2 diabetes is the most common form of diabetes.

It develops when the body’s cells become less responsive to insulin, a process known as insulin resistance.

Initially, the pancreas compensates by producing more insulin. Over time, however, it may struggle to keep up with demand, resulting in elevated blood glucose levels.

Type 2 diabetes is influenced by a combination of genetics, lifestyle factors, physical inactivity, body composition, and environmental influences.

Exercise plays a particularly important role in managing this form of diabetes.

Gestational Diabetes

Gestational diabetes develops during pregnancy when hormonal changes reduce insulin sensitivity.

Although blood glucose levels often return to normal after birth, women who experience gestational diabetes have an increased risk of developing Type 2 diabetes later in life.

Exercise is commonly recommended during and after pregnancy as part of ongoing diabetes risk management.  

Why Exercise Is So Effective for Diabetes Management

Exercise influences many of the physiological systems involved in blood glucose regulation.

Unlike many interventions that target a single mechanism, exercise improves glucose control through multiple pathways simultaneously.

Improving Insulin Sensitivity

One of the most important effects of exercise is improved insulin sensitivity.

When muscles contract during physical activity, they increase their uptake of glucose from the bloodstream. Following exercise, muscle cells become more responsive to insulin, meaning less insulin is required to move glucose into the cells.

This helps lower blood glucose levels and reduces the workload placed on the pancreas.

Importantly, these benefits can occur after just a single exercise session. Following exercise, insulin sensitivity can remain elevated for approximately 24–72 hours, allowing the body to regulate blood glucose more effectively during this period.

Because these effects are temporary, consistency is critical. Current diabetes management guidelines recommend avoiding more than two consecutive days without exercise, as the improvements in insulin sensitivity gradually diminish when regular physical activity is not maintained.

This is one of the reasons why regular movement throughout the week is generally more effective than relying on one or two large exercise sessions.

Increasing Glucose Uptake by Muscles

Skeletal muscle is one of the body’s largest consumers of glucose.

During exercise, muscle contractions stimulate glucose transporters known as GLUT-4 to move to the surface of muscle cells, allowing glucose to enter even when insulin levels are lower.

This means exercise can help lower blood glucose both during and after activity.

Regular physical activity effectively creates a larger “sink” for glucose utilisation throughout the body, helping improve blood glucose control and reduce insulin demand.

Improving Body Composition

Exercise helps preserve and build lean muscle mass while supporting healthy body composition.

More muscle tissue means:

  • Greater glucose storage capacity

  • Improved metabolic health

  • Better insulin sensitivity

These adaptations are particularly valuable for individuals with Type 2 diabetes or insulin resistance.

Supporting Cardiovascular Health

People living with diabetes are at increased risk of cardiovascular disease.

Exercise can improve:

  • Blood pressure

  • Cholesterol levels

  • Blood vessel function

  • Cardiorespiratory fitness

These adaptations help reduce the long-term cardiovascular risks associated with diabetes.

Reducing the Risk of Complications

Poorly controlled diabetes can contribute to complications affecting:

  • The heart

  • Kidneys

  • Eyes

  • Nerves

  • Circulation

By improving blood glucose management and cardiovascular health, exercise plays an important role in reducing the risk of these complications over time.

What Type of Exercise Is Best?

The strongest evidence supports a combination of aerobic exercise and resistance training.

Aerobic exercise helps improve:

  • Cardiovascular fitness

  • Blood glucose utilisation

  • Insulin sensitivity

Examples include:

  • Walking

  • Cycling

  • Swimming

  • Jogging

Resistance training helps:

  • Build and maintain muscle mass

  • Improve glucose storage capacity

  • Enhance metabolic health

Examples include:

  • Weight training

  • Resistance bands

  • Bodyweight exercises

  • Clinical Pilates

Both forms of exercise provide unique benefits and are most effective when combined.

How Much Exercise Is Recommended?

Current guidelines generally recommend:

  • At least 150 minutes of moderate-intensity aerobic exercise per week

  • Strength training on 2–3 days per week

Ideally, these sessions should be spread across the week rather than concentrated into a small number of days. This helps maintain the positive effects of exercise on insulin sensitivity and blood glucose regulation.

Importantly, benefits can occur well before these targets are reached. Even small increases in physical activity can improve blood glucose control and overall health outcomes.

For many people living with diabetes, the most important goal is not perfection — it is consistency. Regular movement performed week after week provides the greatest long-term benefit for both glucose management and overall health.
