Pilates for Back Pain – Why Core Stability Matters

Pilates is often recommended for back pain but what does the evidence really say? At The Active Studio, we focus on delivering Pilates that’s grounded in science, not fads. As Accredited Exercise Physiologists, we use targeted core stability training, informed by clinical research, to help manage and prevent lower back pain.

Understanding Lower Back Pain and Core Stability

Lower back pain is one of the most common musculoskeletal complaints, affecting up to 80% of adults at some point in their lives. While it can arise from many causes, from sedentary lifestyles to injury or postural loading, a common contributing factor is poor core control and motor patterning. That’s where core stability training, such as Pilates, comes in. But let’s be clear“core” doesn’t mean six-pack abs or endless crunches. True core stability refers to the deep stabilising muscles like the transversus abdominis, multifidus, pelvic floor, and diaphragm which work together to support the spine and control movement.

What the Research Says

  • People with chronic lower back pain often show delayed activation of the deep abdominal muscles, particularly the transversus abdominis (Richardson et al., 1999).
  • A 2014 systematic review published in Physical Therapy found that core stability exercises are more effective than general exercise in reducing pain and improving function in chronic low back pain.
  • Pilates-based interventions have shown improvements in pain, function, postural control, and movement patterns, particularly when supervised by trained professionals (Wells et al., 2014).

In fact, over-bracing or excessive tension can worsen back pain. We teach clients how to:

  • Activate the deep core in a coordinated, automatic way
  • Breathe diaphragmatically and use the core as a dynamic system
  • Move efficiently through real-life tasks like lifting, bending, or walking
