Heart Health and Stroke: How Exercise Can Reduce Your Risk

Heart disease and stroke remain two of the leading causes of death and disability worldwide. While they are often thought of as separate conditions, they are closely connected, sharing many of the same underlying risk factors and physiological processes.

The good news is that many of these risk factors are modifiable. Regular exercise is one of the most effective interventions for improving cardiovascular health, reducing stroke risk, and supporting long-term wellbeing.

Understanding the Link Between Heart Disease and Stroke

Your heart and blood vessels form a single interconnected system responsible for delivering oxygen and nutrients throughout the body.

When this system becomes damaged, the risk of both heart disease and stroke increases.

A stroke occurs when blood flow to part of the brain is interrupted, either by a blockage (ischaemic stroke) or bleeding (haemorrhagic stroke). Around 80–85% of strokes are ischaemic, often resulting from narrowed or blocked arteries.

Similarly, heart attacks occur when blood flow to the heart muscle becomes blocked.

Both conditions are frequently caused by the gradual build-up of fatty deposits, known as atherosclerosis, within the arteries.

The healthier your cardiovascular system, the lower your risk of both conditions.

 

 

Exercise Improves the Health of Your Blood Vessels

One of the earliest and most important adaptations to regular exercise occurs within the blood vessels themselves.

The inner lining of blood vessels, called the endothelium, plays a critical role in regulating blood flow and blood pressure.

Regular physical activity improves endothelial function by increasing the production of nitric oxide, a naturally occurring molecule that helps blood vessels relax and widen.

This leads to:

  • Improved blood flow
  • Reduced vascular resistance
  • Better oxygen delivery
  • Lower blood pressure

Healthier blood vessels are less likely to become damaged and less likely to develop the plaques that contribute to heart attacks and strokes.

 

 

Lowering Blood Pressure

High blood pressure is one of the largest modifiable risk factors for both heart disease and stroke.

Over time, elevated blood pressure places excessive force on the walls of the arteries, causing damage that accelerates plaque formation and increases the likelihood of blood vessel rupture.

Regular exercise helps lower resting blood pressure through several mechanisms, including:

  • Improved blood vessel flexibility
  • Reduced peripheral vascular resistance
  • Increased stroke volume
  • Improved autonomic nervous system regulation

Research consistently shows that aerobic exercise combined with resistance training can produce clinically meaningful reductions in blood pressure, making exercise one of the first-line treatments for hypertension.

 

 

Improving Cholesterol and Blood Lipids

Exercise also improves the balance of fats circulating within the bloodstream.

Regular physical activity can:

  • Increase HDL (“good”) cholesterol
  • Reduce LDL (“bad”) cholesterol
  • Lower triglyceride levels

HDL cholesterol helps transport excess cholesterol away from the arteries and back to the liver for processing, slowing the progression of atherosclerosis.

Over time, these changes help protect the arteries supplying both the heart and brain.

 

 

Improving Blood Sugar Control

Diabetes significantly increases the risk of both cardiovascular disease and stroke.

Following exercise, skeletal muscle becomes more sensitive to insulin, allowing glucose to move from the bloodstream into muscle cells more efficiently.

This increased insulin sensitivity can remain elevated for up to 24–72 hours after exercise, highlighting the importance of exercising regularly rather than only occasionally.

Improved blood glucose control reduces damage to blood vessels and lowers one of the major contributors to cardiovascular disease.

 

 

Strengthening the Heart

Your heart is a muscle, and like any muscle, it adapts to regular training.

Consistent aerobic exercise increases stroke volume, meaning the heart pumps more blood with each beat.

As a result:

  • Resting heart rate decreases
  • Cardiac efficiency improves
  • Oxygen delivery increases
  • Everyday activities require less cardiovascular effort

A stronger, more efficient heart places less strain on the cardiovascular system over time.

 

 

Reducing Inflammation

Chronic low-grade inflammation contributes to the development of atherosclerosis and cardiovascular disease.

Regular exercise helps regulate inflammatory processes by reducing inflammatory markers while improving immune system function.

This creates a healthier internal environment that supports long-term cardiovascular health and slows disease progression.

 

 

Managing Body Weight and Physical Capacity

Exercise also helps maintain a healthy body composition.

Excess body fat—particularly around the abdomen—is associated with increased blood pressure, insulin resistance, abnormal cholesterol levels, and chronic inflammation.

What Type of Exercise Is Best?

The strongest evidence supports combining both aerobic and resistance training.

Aerobic exercise such as:

  • Walking
  • Cycling
  • Swimming
  • Jogging

improves cardiovascular fitness and blood vessel health.

Resistance training helps improve:

  • Muscle strength
  • Insulin sensitivity
  • Functional capacity
  • Metabolic health

Together, these forms of exercise produce greater overall cardiovascular benefits than either alone.

 

 

How Much Exercise Is Recommended?

Current Australian and international guidelines recommend:

  • At least 150–300 minutes of moderate-intensity aerobic exercise each week, or 75–150 minutes of vigorous-intensity exercise
  • Strength training at least two days per week
  • Limiting prolonged periods of sitting by incorporating regular movement throughout the day

If you’re currently inactive, it’s important to remember that some exercise is always better than none.

Even small increases in physical activity can significantly reduce cardiovascular risk.

 

 

Prevention Is More Powerful Than Treatment

Many of the major risk factors for heart disease and stroke develop silently over many years before symptoms appear.

Exercise offers the opportunity to intervene early.

By improving blood pressure, cholesterol, blood glucose, cardiovascular fitness, vascular function, and overall physical capacity, regular movement helps address many of the underlying causes of cardiovascular disease—not simply the symptoms.
